Currently, all appointments are conducted virtually through a Zoom platform integrated through Practice Better. You will receive a link to the appointment as well as reminders upon booking.
To deliver the best possible care I recommend you complete the following forms and provide the following labs (if possible):
Client informed consent, medical history questionnaire, medical symptom questionnaire, 3-day food journal (to be sent via the electronic medical record platform, Practice Better)
Any medical testing or notes (within the last 3 months)
A list of symptoms, concerns, and questions you may have for me

We will take a deep and comprehensive dive into your past and current health concerns through a multifactorial lens. We will discuss your nutrition, lifestyle, family medical history, pharmacological and supplemental use. In addition, we will go through with a fine-tooth comb and discuss potential triggering events as well as mediators and perpetuators to your current health concerns. We will also discuss whether the use of functional medicine labs would be of benefit to journey towards health optimization.
At the cessation of this session, we will have created an initial actionable plan (this includes nutrition, exercise, supplementation and lifestyle interventions) that you may start integrating into your daily life to start moving you towards your health goals of living life #fullyoptimized.

During these visits, we will revisit your symptoms and quantify and compare improvements based on the medical symptom questionnaire that I request to have completed from prior to your initial consultation. We will discuss how well your nutrition, supplemental and lifestyle interventions have been assimilated and address any questions or concerns that you may have. These follow-up visits are also integral to success from an accountability stand-point. I can’t tell you the number of times I have been told, “Jamie, I know what to do but I don’t know why I’m not doing it.”
The Portal makes it easy for patients to request appointments, receive care plans based on what was discussed at your sessions, receive supplemental instructions, and much more. The Practice Better platform is also where balances for sessions are paid and invoices for payments are provided. This platform is 100% HIPAA compliant.
The state of a patient’s current condition, level of toxicity, nutrient deficiencies, hormone imbalances and emotional state are all taken into consideration when creating their treatment plan. Because every individual is unique, the length of time for full health restoration may vary widely from one person to the other.
Clients may ask questions through the Practice Better portal. However, if questions require a more in-depth explanation, a Booster Session may be suggested to address the question/concern fully.
Unfortunately, I do NOT bill for insurance.
You can use your HSA or FSA credit card to pay for my services, nutritional supplements and functional medicine testing.
The functional nutrition approach that I use to address root causes is not fully reimbursable through health insurance companies because they still do not recognize the efficacy of nutrition therapy as a modality in the managementmof the conditions I specialize in.
Under the current conventional or allopathic medical model, insurance companies structure their reimbursement around drug prescriptions & surgeries. Therefore, this model does NOT incentivize healthcare practices that support root-cause solutions, which the functional medicine model does. Functional medicine/functional nutrition is considered to be ‘above the standard of care’. Hopefully one day it will be THE standard of care.
For now, I have opted out of insurance plans to provide better services to you, to help you actually feel better!
